Consumer Health Data Privacy Policy
Effective 1 October 2026. This policy is provided under the Washington My Health My Data Act and Nevada law. It covers consumer health data only. Our general Privacy Notice covers everything else.
Consumer health data we collect, and why
When you submit a Booking Inquiry on sylkhealth.com we collect the medical procedure you select, your email address, your preferred language, and any message you choose to add. We collect it to send your inquiry to the provider you selected so that provider can respond to you. We also record the date and time of your consent and a hashed form of your IP address as evidence that you agreed.
Sources of consumer health data
We collect this information directly from you when you complete an inquiry form. We do not buy or receive consumer health data about you from anyone else.
Consumer health data we share, and with whom
We share the procedure you selected, your email address, your preferred language and any message you add with the provider you chose. That provider may be located outside the United States and decides for itself how it uses the information once it receives it. We also share this information with the service providers that host our website and deliver our email, only so that they can operate the site and send messages about your inquiry.
Sale of consumer health data
We do not sell consumer health data.
How to exercise your rights
You may ask us to confirm whether we hold consumer health data about you, to give you a copy, to tell you who we shared it with, to delete it, or to withdraw your consent. Email privacy@sylkhealth.com with the subject line "Consumer health data request". We will confirm your identity by replying to the email address on the inquiry and respond within 45 days. If we deny a request, you may appeal by replying to our decision within 30 days. We will answer the appeal in writing within 45 days and tell you how to contact the Washington Attorney General if you disagree.
Reviewing and changing your information
You may review or correct the information you gave us by emailing privacy@sylkhealth.com.
Changes to this policy
If we change this policy we will update the effective date above. Where a change affects consumer health data we already hold, we will email the address on your inquiry before the change takes effect.
Collection across websites over time
Our website loads Google Tag Manager. Its tags do not run until you allow analytics or marketing cookies in the cookie preferences control. No consumer health data from an inquiry form is sent to any analytics or advertising service.
